Sports Packets
Any student
who wants to try out for a sport at SDA must complete a sports
packet and have it on file in the Athletic Office. In exchange, the
student will receive a blue card signed by the Athletic Director or
Athletic Secretary which he/she must take to tryouts and hand to the
coach in order to participate. Due to increased numbers of
student-athletes participating in SDA Athletics, in some sports not
everyone will make the team. As such, it is important that
student-athletes attend all of the tryout days and times. Sports packets
can be downloaded or are available on
the table outside of the Athletic Office.
Winter Sports Packets are due to the
Athletic Office no later than Nov. 6th.
Make sure to attach a copy of your 1st Quarter Progress Report
card which you will receive in Homeroom on Oct. 9th. Report
cards can also be downloaded from the Parent Portal.
If you submitted a Fall sports packet, you only need to bring in a
copy of your grades.
Blue card pick up begins Nov. 10th during lunch and after school.
JV GIRLS BASKETBALL PLAYERS NEEDED:
Any girl interested in playing basketball, turn in a completed
sports packet to the Athletic Office.

Conference
San Dieguito Academy student
athletes have 18 varsity/junior varsity level sports offered to them. The Academy is a

Eligibility
and Paperwork
To be eligible to try out for a Mustang Team, each
student must:

Pick up a Sports Packet:
Pick up the packet from the Athletics Office or online from the link above left.
- Athletic Emergency Release Form: Complete
the form.
- Preparticipation Physical Evaluation Form:
Obtain a physical examination from doctor. Complete side #1 of Preparticipation Physical Evaluation. Have doctor complete and sign side #2 of Preparticipation Physical Evaluation.
- Use of Private Auto for School Activity:
Complete the form.
- Code of Conduct: Read the Code.
- Acknowledgement of District Hazing Policy:
Read and sign top of this page.

Steroid Policy:
Read and sign.
- Acknowledgement of Receipt of Athletic Handbook:
(Bottom of the same form) The Athletics Handbook
can be found on this website.
Sign acknowledgement regarding Ethics and Sports.
- Transportation fee:
Include a check for district transportation. The fee is $100 per sport for participating in 1, 2 or 3 sports.
There is a cap of $200 per academic school year, per athlete. Checks should be made
payable to: SDUHSD.
- Following completion of paperwork in the Sports
Packet:
- Submit the completed Sports Packet to the
Athletics Office prior to try-outs.
- Receive Eligibility Card: Clearance will
be issued in the form of a blue Eligibility Card.
- Take Eligibility Card to coach:
Take the
blue Eligibility Card to try-outs and hand it to the head coach in order to participate.
- Attend Try-outs: Try-outs are held for a
minimum of 3 consecutive days and athletes are expected to attend all try-out sessions.
- Attend Sports Night: Parents are to
attend seasonal Sports Nights to meet the coaches and become familiar with the athletic
program at SDA.
No Contact Period: This is
a time period where students cannot have any kind of coaching, practice
or game contact with their school coach. This time is designed to
give families a break from athletics and year round competition.

July 26 - August 8, 2010
